Too small for a knee
I did return it as it wasn't appropriate for knee replacement. It's called wrap, but it's actually a pull-on, making it difficult to use after surgery. It appears to be high quality, but I returned it because it didn't work for my purposes.

put a blanket over it so that it only cools you, not the air.
Perfect. Nice and soft. I put it over a light cotton sweater, and I put a wool throw over the sleeve so that it doesnt get warm from the air. Size M for a womans elbow, to go over the light sweater. AHHHH I keep my freezer at 0 degrees

Runs large
I didn't find this to be true to size. I ordered two similar products, and they had a much better fit. This was much looser so it wouldn't have been effective for cold therapy. It did have a comfortable feel to it and the materials seemed to be good quality. Perhaps go down a size if available.
