What waist sizes does this fit?

The range for the hip belt is for waist size 29" to 44"

Could the short velcro be replaced by straps so that it would work with taller kids (the bar could be lower than the waistbelt)?

Yes, of course you can add straps to lower the bar. Do some tests at home before trying it out on your adventures.

Am I buying the exact item that’s on the picture Does it come with the bar where the Chao stands in

The Piggyback Rider Hip Belt is just the Hip Belt which is an accessory for the SCOUT Standing Child Carrier. You need the carrier set to work with the Hip Belt.

What if the kids get tired? Are they able to sit

The Piggyback Rider is the worlds first and only standing child carrier. If the child gets tired they can remove some pressure from their legs by leaning up against the parent, but ultimately cannot sit down.
