does it come in assembly with tripod?

No. This is only the treaded insert for the tripod. It fits common binoculars, spotting scopes, etc. the trigger stick 3 is sold separately. The trigger stick does come with this attachment and also a fork gun rest.

It fits mine very well. Most likely it should fit yours if it is unaltered.

My Primos shooting stick came with one initially and I bought a few extra so that I could quickly swap out devices.
