What is the largest waist size ?????

It's really big, I would say up to a size 18. Basically it's a fairly big heat pad that will cover your lower back, then there is a breathable band with a Velcro type material on one end that sticks to the material so you can have it as tight or loose as you need.

What is the maximum circumference it will expand to?

It's a wrap belt type so I would say a comfy 38 to 40 inch waist comfy to wear.

I find these wraps brilliant. I suffer from a severe back problem, after having a discectomy and ten years later spinal fusion, I’m taking painkillers which really help but not enough, so I use Therma wraps and these are great, within a short time 15 or so minus it almost takes away the pain at least enough to enable me to do most of the things I want to do. I can’t use them as suggested by the company so I cut them so that they just fit on my back and use micropore tape to keep it on. I have never had any problem using them and would be devastated if they were taken off the market. I have used them for more than 15 years.
