Are they flat or a little padded?

They seem a little padded not frozen. When frozen, no padding. Good idea to put a thin towel between you and the ice pack.

I only want the wrap,does anybody just sell the wraps or covers?

lost directions how do you attach it to your shoulder?

Don't know if I have instructions either. check out the picture on the AMAZON link for it. It shows the pad laying on the shoulder with one strap on top area of pad wrapped around the chest under the other arm to secure on top of wrap. And second smaller wrap around bicep area holding wrap around arm under armpit. Hope this helps.

It is flexible enough when frozen?

I used this for my knee - it is pretty stiff when frozen, but i was able to wrap around my knee enough and the straps helped.
