What material is the base made from?

Molded plastic, but it's very sturdy and holds the weight of the phone just fine. I have mine mounted with just the adhesive tape included in the kit.

The 3M tape on my mini dash pad is coming off, and the pad will not stay on,.... how do we get a replacement?

3M double sided tape can be bought at most auto parts stores. Make sure the surface completely clean and grease/oil free....including natural oils from your skin before you apply the tape to either surface. An alcohol wipe works well for this.
