When it says 40mm long, how is that measured?

The 40mm is measured from center to center. In other words, from the center of the handlebar to the center of the steer tube.

Whats the torque for the installation of this stem? On the steering tube and the handlebars clamp.

I just keep rotating around tightening until something snaps then order a new stem and do a half turn less than what caused the last one to snap.
