Does it take much attention or effort to avoid inadvertently clicking the buttons while holding and using it??!

No, it really seems to come naturally. After a day of using - I would highly recommend this. I have severe tennis elbow. Its been about a month of using this mouse. I work in an office at a desk all day clicking and clicking. This mouse has calmed down my inflammation and it doesn't hurt anymore.

Can you "click" the scroll wheel? Or does it only scroll?

How durable is the scroll wheel? My current ergo mouse's scroll wheel has worn out in less than a year.

I've had two of these mice in eight months. First one wore out within four / five months. Got a replacement. That one wore out within three / four months. I really want to love it as it is so comfortable for me...but I rely so much on the wheel.

I am used to Logitech's free scrolling/smooth scrolling wheel. Does this mouse offer smooth scrolling?

No, I owned a Logitech mouse as you describe and miss the scroll wheel every day. I need to use vertical mice, so I cannot go back to Logitech unfortunately (their vertical mouse does not have the free-scroll wheel either). This mouse is pretty loose/smooth feeling for a bump/clicky wheel. Probably once of the better feeling I've used in that regard. However it is a far cry from the Logitech free scroll (as are all mice)
