Will the adaptor work on a USA 2 prong plug or only a 3 prong?

Works on either. The phone/tablet chargers we plugged into it had 2 prong plugs. Make sure that adapters will handle 220 to 110 volt current, though. Most newer ones do, but it will be printed in mice type on the adapter.

Does this plug accept polarized two-pronged plugs? (those with one blade wider than the other)

Yes. I just looked at the one I ordered and one side is clearly larger than the other. I also plugged cell phones, laptops, and other high power electronics and it worked fine for all of them. I imagine at least one of those had to have a polarized plug.

Does it work well with 3-prong US adapters over months of use? (Will be Australia for a few months and need it for charging laptop)

3-prong US adapters fit very well into this converter. I only used my adapter for five weeks, but during that time I saw no indication of fatigue problems or anything that might affect it working for months.
