How do you attach this?

It depends on how you intend to wear it. If you intend to wear it. It you intend to wear it vertically behind your back, then you will want to attach the larger brackets on the side of the sheath. If you intend to wear it horizontally (as I do) then you simply attach one of the smaller clips on the end of the sheath in line with your belt

Will this attach to a 2.25” duty belt?

Belt loops measure 1 3/4 inches.

would this work on a molle backpack and how do you install it?

Only if you have straps to weave through the slots. A flat head screw driver worked for me.
