Does this include screws for mounting to a beam? (Information says "any home with exposed beams or joists" but not clear what hardware is included.)

Yes, mine came with heavy duty wood screws and it's been mounted and working just fine for months now.

once mounted about how far down from the rafter to the bottom of the bar will it be hanging?

We have it mounted to the joists in our unfinished basement. The holes were drilled between 3 and 4 inches from the bottom of the joist. The bar is approximately 11 inches below the bottom of the ceiling joist.

Would the screw mounted bar affect durability of the basement joist over time? If it is used 3 days a week for 3-5 years, could it break the joist?

Depending on a number of variables, including the material the joist is made from, your weight, and the angle the pressure comes from, but I would doubt it. I use my bar as a base for other equipment. Reinforcing the joist, if you are concerned, could be an option.

Does the hardware have to go all the way through and be bolted on the other side? The beam I had in mind is pretty thick

I didn’t. Used the beams in my basement, which are massive. Buy some 2 inch lag bolts and predrill a thinner hole. Bolt in place. I’m 210, do pull-ups with a 45lb weight and haven’t had a problem 👍🏽
