How often do you recommend replacing this pillow?  I mean, the foam can't last forever, right?

This is a great neck pillow. I have had mine for 5 years and am just needing to replace it. 
It doesn’t break down like most pillows.

I have knots in my neck always. Would this foam make my neck more sore? Double Core or Tri Core Excel or Iso Cool so many choices?

The Tri Core Excel helped me for a while, but it broke down pretty quickly.  The quality of these pillows isn't such that they last long.  I eventually ended up with this Iso-cool contour pillow: http://www.amazon.com/gp/product/B000ZK7MPG/ref=oh_details_o00_s00_i00?ie=UTF8&psc=1  It isn't perfect, but it is the best I have found.

what is the foam made of?  Not the cores. The rest of the pillow. Is it polyurethane foam?

I found this on Core Website, Says Foam then explains about the polyurethane foam, so my guess would be polyurethane foam:A.  Fiber:  Allows for comfort and support.  High quality fiber is blown to exact specification to insure uniform distribution and pillow weight.    Originally used to emulate features of natural down pillows & used in comfort for accommodation.    Advances in fiber have resulted in great ability to comfort & support.    Core Pillows use conjugated fiber which acts like a spring.     During manufacturing, high pressure will link fibers together & different resiliency levels can be created in the pillow.  This process helps fiber stay in place and resists migration, yielding consistent support as fiber generally has amorphous tendencies and can clump or rat tail during cleaning.    Foam can offer the most consistent support but Fiber currently can offer almost as much but with more comfort.    Fiber material is available in wide array of sizes, shapes & coatings.    Has ability to perform as either a semi-solid or semi-fluid.    Has natural flame retardance & durability.B.  Polyurethane Foam:  Has memory to return to original shape.  All of the foam used can be classified by two measurements: 1.  Density and 2. Indent Load Deflection (ILD). Foam pillows have been around for quite some time.
