You can use this item as a heat sourced or a cold source, which is great. Even though this product does have some stretch ability, it does run VERY small, so unless you are young or quite thin in bone structure, you won't get it past your foot up onto your knee. I found that out immediately when I purchased the first one I ordered. BUT I was surprised to discover that it did fit my ankle very well, and curved right on there and stayed put, with no problem. I've had issues finding anything that would give me some type of relief for my swollen 20" ankles! I ordered this one, so that I'd have one for each foot. It gives great cold relief to my ankle, is very easy to freeze and reuse, and was very reasonably priced. I can get up and walk inside the house with it on. Best to wear a sock and then put it on over the sock. It's made very well and I've been using it.
